The SOS program helps chapters improve through sharing and finding easy-to-use ideas, resources, and practices. Thank you for taking the time to share your chapter's success. Prior to starting the submission, read through this form, and collect any documentation you might want to submit.

Please complete the entire form and submit for consideration by the Chapter Recognition Committee (CRC). Should additional information be needed, a member of the CRC will contact you directly. When responding to each area, please provide information about who, what, when, how, why, where, and to what degree where appropriate. Also, please be specific and provide enough detail that would allow a leader from a different chapter to pick up this document and mimic your effort.
